✔️ Original copy of the form related to the grant should be submitted and this form should be submitted with the signature of His Excellency the President during all the period from the date of first registration to the present time of registration.

✔️ Enjoyment of Life National Identity Card certified by the Grama Niladhari of the applicant.
✔️ In case of non-registration of births, marriages and deaths, a formal affidavit should be submitted along with the blank results of the search. (An affidavit certified by a Justice of the Peace signed on Rs. 50 / = stamps and a Grama Niladhari report should be submitted to confirm the information contained in the blank result sheet. Must be signed and submitted.)

Request for permission to acquire land / part of land
✔️ Request letter from the Divisional Secretary for the transfer of land should be certified and submitted by the Grama Niladhari. If the whole land is to be transferred, that is, and if a portion of the land is to be transferred, the extent of the land to be transferred should be included in the request letter.
